Inclusion and exclusions for the gentamicin calculator for ONCE daily regimens are as follows:

Inclusion Criteria

  • Adult patients with normal renal function (CrCl > 20ml/min) who are not under the care of the critical care team

Exclusion Criteria

Gentamicin should not be used in the following patient groups:

  • Myasthenia Gravis
  • Hypersensitivity or allergy to aminoglycosides (these include amikacin, gentamicin, neomycin, streptomycin and tobramycin)

ONCE daily gentamicin should not be used for the following indications/patient groups:

  • Infective Endocarditis (refer to Gentamicin Prescribing on Cardiothoracic Wards)
  • Cystic Fibrosis
  • Pregnancy
  • Paediatrics
  • Patients with a CrCl less than 20ml/min (refer to Guideline for Antibiotic Use in Patients with Renal Impairment)
  • Patients under the care of the critical care team (refer to Critical Care Guidelines for Gentamicin)
  • Patient with major burns (>20% of the total body surface)
  • Patients with ascites
